APACHE II Score

Used to assess disease severity and mortality risk in ICU based on physiological indicators, age, and pre-existing conditions.

An observational scale used in patients unable to communicate their pain. Used to assess pain in children aged 2 months to 7 years, including those with developmental delays; excluding paralyzed patients.

FOUR Score

Assesses the state of consciousness with a focus on brainstem reflexes and breathing patterns for detailed neurological evaluation.

Glasgow Coma Score (GCS)

A fundamental tool for monitoring neurological status. Used to assess the level of consciousness after traumatic brain injuries or neurological disorders.

HAS-BLED Score

Designed to assess the risk of major bleeding within a year in patients taking anticoagulants for atrial fibrillation (AF).

Modified Aldrete Score

The Modified Aldrete Score is used in post-anesthesia care units (PACU) to assess when a patient has recovered sufficiently from anesthesia to be safely discharged to a regular hospital ward.

PESI Score

PESI is intended for risk stratification (assessing 30-day mortality) in patients diagnosed with PE to determine the severity of their condition.

Phoenix Sepsis Score

Used to monitor the severity of organ dysfunction during intensive care of sepsis in children.

qSofa Score

The Modified Rankin Scale (mRS)

Commonly used scale for measuring the degree of disability or dependence in the daily activities of people who have suffered a stroke or other causes of neurological disability.

Wells Score

A scale for assessing the clinical probability of pulmonary embolism, used in stable patients (without shock or hypotension) with suspected PE.
