The Modified Rankin Scale (mRS)

Commonly used scale for measuring the degree of disability or dependence in the daily activities of people who have suffered a stroke or other causes of neurological disability.

Patient's Activity Level	No symptoms at all - 0 point Despite symptoms, able to carry out all usual duties and activities - 1 point Unable to perform all previous activities but able to look after own affairs without assistance - 2 point Requiring some help but able to walk without assistance - 3 point Unable to walk without assistance and attend to own bodily needs without assistance - 4 point Bedridden, incontinence, and requiring constant nursing care and attention - 5 point Death - 6 point

Standardized Interview for the mRS: Ask these Yes/No Questions: - Do you have any symptoms that are bothering you? - Are you able to do the same work as before? - Are you able to keep up with your hobbies? - Have you maintained your ties to friends and family? - Do you need help making a simple meal, doing household chores, or balancing a checkbook? - Do you need help with shopping or traveling close to home? - Do you need another person to help you walk? - Do you need help with eating, going to the toilet, or bathing? - Do you stay in bed most of the day and need constant nursing care?
