Full Outline of UnResponsiveness (FOUR) Score

Assesses the state of consciousness with a focus on brainstem reflexes and breathing patterns for detailed neurological evaluation.

Glasgow Coma Scale (GCS)

A fundamental tool for monitoring neurological status. Used to assess the level of consciousness after traumatic brain injuries or neurological disorders.

Pediatric Glasgow Coma Scale (pGCS)

Used to assess the level of consciousness in children aged 2 years and under. For those older than 2 years, use the standard Glasgow Coma Scale (GCS).

Phoenix Sepsis Score

Used to monitor the severity of organ dysfunction during intensive care of sepsis in children.

PESI is intended for risk stratification (assessing 30-day mortality) in patients diagnosed with PE to determine the severity of their condition.

SMART-COP Score for Severe Pneumonia

Designed to identify individuals who require intensive respiratory or vasopressor support (IRVS) support due to pneumonia but can be used only for patients older than 18.

Wells Score for Pulmonary Embolism

A scale for assessing the clinical probability of pulmonary embolism, used in stable patients (without shock or hypotension) with suspected PE.
