Clinical context

Anion gap reference intervals are assay- and lab-dependent (often around 3-11 mmol/L with modern analyzers). Interpret against your local laboratory range. Delta-ratio (delta-gap) interpretation is a bedside aid, not a stand-alone diagnosis: • <0.4: usually suggests predominant normal-anion-gap (hyperchloremic) acidosis • 0.4-0.8: mixed high-AG + normal-AG acidosis can be present • 1.0-2.0: often consistent with predominant high-AG acidosis • >2.0: consider concomitant metabolic alkalosis or chronic respiratory acidosis Always integrate clinical context, lactate/ketones, renal function, and albumin correction.
