Clinical context

This calculator returns a user-selected weight-based dose of intravenous fentanyl for anesthesia or acute procedural analgesia. It does not apply to transdermal fentanyl or chronic-pain conversion. Fentanyl can cause profound respiratory depression, apnea, bradycardia, hypotension, and skeletal-muscle or chest-wall rigidity, particularly after rapid administration or larger doses. Concomitant benzodiazepines, other CNS depressants, and CYP3A4 inhibitors can increase sedation and respiratory toxicity. Dose reduction may be required in older, debilitated, or physiologically compromised patients. Administration requires personnel trained in potent opioid use, continuous respiratory and cardiovascular monitoring, and immediate access to oxygen, airway and ventilation equipment, resuscitation drugs, and an opioid antagonist. Continue monitoring after administration because peak respiratory depression may be delayed.
